Reform measures of nursing vertical management under the background of diagnosis related groups

To summarize the reform measures of nursing vertical management in our hospital under the background of diagnosis related groups,including refined performance management,cancellation of nursing main pharmacy classes,implementation of attending nursing working group,establishment of DRGs nursing quality control coder position,head nurse responsible for bed allocation,deepening nursing quality management and other measures,so as to provide references for other hospitals to carry out the reform of nursing vertical management under the background of DRGs.

Positive psychological experience of stroke patients:a qualitative meta-synthesis

Objective To analyze and evaluate the positive psychological experience of stroke patients to provide reliable bases for their psychological nursing. Methods Chinese and English databases,such as PubMed,Web of Science,Embase,CINAHL,PsycINFO,CNKI,Wanfang Database,VIP,and CBM,were searched,and the literature on the qualitative research on the inner positive psychological experience of stroke patients was retrieved. The time limit of the retrieval was from the establishment to April 2021. The literature quality was evaluated using the 2016 Australian JBI Evidence-Based Health Care Center Quality Evaluation Criteria for Qualitative Research,and the results were consolidated using meta integration method. Results A total of 14 studies were included,and 65 results were extracted and summarized into 12 new categories,which were combined into 3 integrated results. (1)Positive coping attitude and psychology:accepting and adapting to stroke,feeling lucky and grateful,optimistic and positive to face the situation,strong spiritual strength,goals and hope for the future;(2)Positive changes after stroke:increasing health awareness,more intimate and harmonious interpersonal relationships,personal growth,new thinking about the philosophy of life;(3)Support factors of positive psychology;positive coping strategies at the individual level,social support from family,friends and peers,social support from the medical system. Conclusion Existing evidence suggests that patients with cerebral apoplexy after stroke will have a positive psychological experience and change. Nursing staff should pay more attention to the patient’s psychological state and the support of the positive psychology factors,meet their psychological needs,help patients strengthen social support system,and maintain a positive mental state,in order to promote patients to recover and improve the quality of life.

Hazard vulnerability analysis and management strategies of nursing management during the coronavirus disease 2019

Objective To analyze the risks in a temporarily renovated ICU at Tongji Hospital in Wuhan using the Hazard Vulnerability Analysis based on the Kaiser Model,and to propose strategies to respond and manage the risks. Methods The risk management index evaluation system for nursing management was developed based on the Kaiser Model and the status of the temporarily renovated ICU at Tongji Hospital. It was surveyed via web questionnaires during Feb 21st to 25th,2020 among the medical teams in the ICU,and the index risk value and matrix distribution were analyzed. Results The top 13 risk events perceived by the medical team were listed as following.C8 nurses needle stick injury(0.447 6),B1 patients need to be rescued (0.413 0),C10 nurses being infected (0.411 0),B2 patients’ psychological problems(0.407 4),C9 doctors being infected(0.359 8),E3 insufficient or interrupted oxygen supply(0.329 1),D9 drug shortage(0.324 2),E2 insufficient protective equipment(0.317 9);C12 doctors and nurses’ absence from work due to emergency illness(0.309 1),C7 doctors needle stick injury(0.298 2),E1 insufficient supporting staff in the hospital(0.293 5),B6 adverse drug reactions in patients(0.292 4),C6 nurses’ psychological maladjustment(0.289 1). Conclusion Managers should pay attention to the risk assessment on the status of the temporarily renovated ICU at Tongji Hospital,and then guide the problem solving; at the same time,they should attach great importance to the occupational protection and physical and mental health of medical staff.
